Bonus Based on Performance

I am looking for ideas for a Customer Service Rep's bonus program for the new year. What are you basing the bonus on for both the individual effort and the team effort?

if someone from the team has outstanding performance recommended by customer,give him/her bonus based on company's financial availablity.
Also performance is outstanding by team, to do the same thing to this team.
